Molina Healthcare has began using the Texas Standardized Form.  A representative from Molina says they will accept both the previous Molina form and the Texas Standardized form; however, only the Texas Standardized form will be available online and is pre-filled with Molina’s contact information.  The new form has been added to this website for immediate use.
